My son A. Sai Nikhil, aged 20 years studied in 4th Year B. Tech. His ACL tear surgery in your hospital on 11/7/2017 by Dr. B. Chandra sekhar. The told to 6 weeks bed rest. Now, we want to doctor certificate for 6 weeks. We are coming from ESIC medical hospital.

Where we take doctor certificate from you or ESIC.
